Jerry Bouchard

Jerry Bouchard, (53) of Coventry, passed away peacefully at home on Sunday November 16, 2025 surrounded by his loving family and friends. 

Jerry was born in Warwick, RI and graduated from Coventry High School.  He is predeceased by his dad, Robert A. Nelson, Sr. and his grandparents, Michele and Colombe DeCiantis.  He leaves behind his mother, Joyce M. (DeCiantis) Nelson of Coventry and his brothers, Jason M. Bouchard of Coventry, Robert (Dolly) Nelson Jr., of Marlow, Oklahoma and Randy Nelson of Warwick, as well as his beloved dog, Daisy.  In addition to his immediate family, Jerry leaves behind many nieces and nephews, aunts and uncles, cousins, friends, roommates and caregivers from over the years. He was fortunate enough to spend the last eight months of his life in his family home surrounded by his closest family and friends.   

Jerry enjoyed many years of cross country family vacations, riding his bike and spending time with his family and many friends. He participated in Special Olympics as well as supported work programs throughout his life.  He loved to listen to MC Hammer, play video games, watching SpongeBob and the Game Show Network. 

Relatives and friends are kindly invited to attend visiting hours on Saturday, November 22, 2025, from 9:00–11:00 a.m. at the PRATA-GALLOGLY Funeral Home, 78 Providence Street, West Warwick, followed by a funeral service at 11:00 a.m. Interment will take place at St. Mary Cemetery, West Warwick immediately afterward.
